client intake form on this page so your Ayurvedic Practitioner has the necessary background information to give you personalized recommendations.
After filling out the form,
you will be redirected to the Calendar where you can book your Zoom video meeting at a date and time of your preference.
If you cannot make it to your consultation at your chosen time and date, you can email us to reschedule your appointment. Please do so minimum 24 hours before your scheduled meeting, since our Ayurvedic Practitioners' time is in high demand. If you want to cancel your consultation completely, please email us minimum 48 hours before your consultation. Your refund will be initiated within 2 days of cancellation confirmation and will be credited back to you within 7 working days. You will receive your refund in full with no deductions.
